Establish that the people involved are able to bring the right level of thinking skills to the issue. Do they clarify the problem and contract for a common purpose?

Do they ensure the right skills and thinking are available to everyone who needs to be involved in addressing the crisis?

Are the people involved prepared to think collaboratively to get things done?

Are there signs of weak, sloppy or undisciplined thinking – unchecked assumptions, knee-jerk reactions, the wrong remedy applied to the problem, etc.?

Have the people done the required preparatory work to be successful – gathered all the relevant information, found the root cause, and demonstrated flexibility in their thinking?

 
Check to see if the problem is inherent in the execution itself. “Are we shooting ourselves in the foot?” Do the people involved in the process have the technical skills required to have avoided the crisis?

Do the people involved have the necessary resources, including time, to do what’s expected and avoid/prevent similar crises?

Do the people involved have the confidence to use their best judgment and avoid the crisis?

Do those involved have a shared view of what constitutes a successful outcome and sustain a common focus?
